Physcial Therapist - Swing Beds
Full time Physical Therapist position for a 25-bed critical access hospital swing bed unit managing an acute and short-term skilled therapy caseload. Please consider this position if you are interested in these job tasks and have the qualifications described.
Job tasks:
Collaborate with interdisciplinary team to coordinate care and safe discharge
Evaluate/facilitate appropriate admissions w/ the admissions team
Participate in weekly interdisciplinary care plan meetings
Participate in daily unit rounds w/ medical and interdisciplinary team
Develop and facilitate progression of individualized therapy plan of care
Delegate treatment to PTAs
Coordinate treatment with OTs, SLPs, nursing and attending medical provider
Occasional outpatient caseload broad variety of conditions
Qualifications:
NC Physical Therapist License Required. Broad base of clinical skills. SNF experience preferred, knowledgeable of SNF rehab documentation/rehab regulations. Must be team oriented. Program development capabilities. Must foster and implement quality improvement. Must work collaboratively in diverse teams. Must practice excellent customer service. Needds to be knowledgeable in best practices for general medical, orthopedic and neurologic conditions. Must be efficient with use of Electronic Medical Record Systems and proficient w/ Microsoft Office web-based applications. Must be self-directed with the ability to prioritize multiple tasks. Must have innovative problem-solving capabilities.
